[CUCBC Captains] May Bumps 10-outings declaration form

Dear All,

16 forms returned, plenty more still to come. Please remember that one 
form must cover the whole club (both genders), and that the deadline is 
this Wednesday. I'd appreciate it if you could also include your college 
name in the email header when responding, as it makes keeping track of 
things rather easier.
